Acknowledgment of Risk:
I, the undersigned, acknowledge that I am voluntarily participating in this craft workshop, event or activity (“Workshop”) offered by a representative of The Artist Mill, LLC and Flowerful Theory. I understand that the activities involved in this class may include, but are not limited to the use of scissors, glue, sharp tools, ropes, wires, needles and other potentially hazardous materials. Activities will also include direct physical contact with a variety of flowers and plants which may cause allergic reactions. Participants may be offered courtesy flower-infused teas or other snacks and refreshments.
Although Workshop experiences may be used within the context of health and wellness, the activities are educational in nature and provided only as general information and are not medical or psychological advice. The Artist Mill, LLC and Flowerful Theory do not make any representation that you will experience any particular outcome on an issue.
I, the undersigned, understand and acknowledge that participation in this class involves inherent risks, including the risk of injury and allergic reactions. I voluntarily assume full responsibility for any risks of loss, property damage, or personal injury that may be sustained by me as a result of participating in this class. I agree to consult with my health care providers for any specific medical, emotional, or psychological problems or issues in advance of the Workshop. Further I agree to take full responsibility for my self-care during the Workshop and after.
Release and Waiver:
I understand that my participation in the Workshop, including consumption of courtesy refreshments and snacks, is strictly voluntary, at my own risk, and that I freely choose to participate. In consideration of being permitted to participate in the Workshop, event or activity, I, on behalf of myself, my heirs, executors, administrators, and assigns, hereby release, waive, discharge, and covenant not to sue The Artist Mill, LLC, Flowerful Theory, its owners, officers, employees, agents, and representatives (collectively referred to as "Releasees") from any and all liability, claims, demands, actions, and causes of action arising out of or related to any loss, damage, or injury, including death, that may be sustained by me, or to any property belonging to me, while participating in the class.
Indemnification:
I represent that I am competent and able to understand the nature and consequences of participating in the Workshop, and that I am an adult under the laws of the State of my residence. I agree to indemnify and hold harmless the Releasees from any loss, liability, damage, or costs, including court costs and attorneys' fees, that they may incur due to my participation in the class, whether caused by the negligence of Releasees or otherwise.
Medical Treatment:
I consent to receive medical treatment deemed necessary if I am injured or require medical attention during my participation in the class. I understand and agree that I am solely responsible for all costs related to such medical treatment, medical transportation, and any other related expenses.
Food and Drink Allergies:
I understand that food and beverages may be offered during the Workshop. I acknowledge that it is my responsibility to inform The Artist Mill, LLC and Flowerful Theory of any food allergies or dietary restrictions I may have prior to the event. I agree to assume all risk associated with consuming any food or drink provided. I release, waive, and discharge the Releasees from any liability related to allergic reactions or any other adverse effects that may arise from my consumption of food or beverages provided during the class.
Photography/Videography Release (OPTIONAL):
I grant permission for a designated representative of The Artist Mill, LLC and Flowerful Theory to use photographs and/or video recordings of me taken during the class for promotional purposes, including on their website and social media platforms.

Governing Law:
This waiver and release of liability shall be governed by and construed in accordance with the laws of the state of California.
